Linda Fukuda Family Dentistry offers a variety of payment methods.  Unfortunately, not every patient has dental insurance to help cover the costs.  For those patients who do, we are happy to process the required information needed for reimbursement by your dental insurance carrier (this is often done electronically).  Whatever portion your dental plan does not cover, your are responsible for the remainder.  Unless prior financial arrangements have been made with our office manager, your portion will be due upon receipt of our statement.  For those who do not have dental insurance, we accept cash, checks, Visa, Mastercard.  In addition, we offer dental financing through Care Credit, which can be a very helpful option, especially if more than just routine treatment is recommended.  If Care Credit is not an option for you, we ask that you inquire on making payment arrangements prior to scheduling any dental appointment. Below you will find a few of the dental insurance companies that we participate with.  If you do not see your dental plan listed, please give our office a call as you may still be eligible for coverage at our office.
IMPORTANT: Many of the Insurance companies below offer HMO or DMO plans for which we do not accept. Those we have listed with an asterisk, offer such plans.  We ask that you carefully review your benefit booklet or contact your HR department or insurance company directly, to clarify whether or not this applies to your plan.
